Abstract |
Peptoniphilus harei is Gram-positive anaerobic coccus which is a commensal organism of the human skin, vagina, and digestive tract. There have been some case reports describing human infections caused by P. harei, however few case series have been published. The objective of this study was to retrospectively examine the clinical features of P. harei bacteremia in our hospital. We included all patients with P. harei-positive blood cultures who had been treated between June 2005 and May 2016 at Kameda Medical Center, a 900-bed tertiary care hospital in Japan. We collected demographic and clinical data. During the study period, we experienced 7 cases of P. harei bacteremia. Six cases were female. The median age was 71 years (range, 30-92 years). All cases had underlying disease in the genitourinary tract. The diagnosis was endometritis (n=3), pyelonephritis (n=3) and intraabdominal infection (n=1). Three isolates were identified using 16S ribosomal RNA sequencing, and four isolates were identified using Matrix Assisted Laser Desorption/Ionization Time of Flight Mass Spectrometry (MALDI-TOF MS). Three had polymicrobial bacteremia. Median duration of antibiotics was 14 days (range, 7-23 days). 30-day mortality was 0%. In our hospital, P. harei bacteremia was observed mainly in female and patients with underlying diseases in the genitourinary tract, and originated mostly from endometritis and pyelonephritis. P. harei bacteremia may be underestimated because identification is difficult by conventional method and MALDI-TOF MS can be an efficient technology for the identification of P. harei. P. harei can be an important pathogen in anaerobic environment due to genitourinary abnormality. |
