Abstract We encountered a case of sepsis caused by Leptotrichia hongkongensis. A 1-year-old girl was admitted to our hospital with fever. The blood culture turned out to be positive in the child bottle. Fusiform gram-negative rods were observed in a gram stain of the blood culture, although no bacterial growth was observed in the subcultures. Then, another blood culture was carried out by adding the first positive culture to human blood and inoculating new blood culture bottles with this mixture. In the second blood culture, the anaerobic bottle was found to be positive after 46 h of incubation. Bacterial growth was observed after anaerobic cultivation for 48 h on sheep blood agar / chocolate agar medium (Nissui Pharmaceutical) and Anero Columbia RS ager medium (Becton Dickeinson and Company). The isolated bacteria were catalase test negative, α-galactosidase negative, β-galactosidase negative, α-glucosidase positive, and β-glucosidase positive. The isolated colony could not be identified with a MALDI Biotyper mass spectrometer (Bruker Daltonics), but was identified as L. hongkongensis via 16S rRNA genetic analysis. Based on the results of the gram staining, biochemical tests, and 16S rRNA genetic analysis, we comprehensively identified the bacteria as L. hongkongensis.
