Abstract Herein, we report the first case of diverticulitis due to Comamonas kerstersii in Japan. A 50-year-old man who had a fever over the previous day, shaking chills on the day of hospitalization, conjunctival hyperemia and reduced blood pressure was urgently transported to our hospital. Two aerobic bottles of two sets of blood cultured on the day of hospitalization were positive the next day. The gram staining findings of positive blood cultures showed glucose non-fermentative gram-negative bacilli. They were identified as C. kerstersii by direct identification from positive blood culture using MALDI-TOF mass spectrometry, and the patient was rapidly diagnosed as having diverticulitis from the bacterial information. Although this bacteria can be identified by MALDI-TOF mass spectrometry or 16S rRNA gene analysis, there are no databases in the automatic bacterial identification systems currently available, and it may be confused with C. testosteroni. Therefore, confirmation of the biochemical test is important for the correct identification. The direct identification from a positive blood culture using MALDI-TOF mass spectrometry and rapid reporting of bacterial information in the present case were useful for diagnosis.
