Abstract A 30-year-old woman, who had been using steroids and immunosuppressants for the treatment of dermatomyositis, presented with fever and subcutaneous nodules and nodular erythema nodosum on both legs during follow-up. After admission, the dosage of steroids was increased but her symptoms did not improve. Therefore, on the 6th day post-admission, left knee exudate, blood, and induced inhaled sputum were collected. Intratracheal sputum and exudate were positive for Ziehl-Neelsen staining. Fever and exanthema were mitigated by treatment with clarithromycin (CAM), ethambutol (EB), isoniazid (INH; administration was discontinued after two days), and rifampicin (RFP). However, fever recurred on the 14th day since the start of treatment and the relapse of dermatomyositis, drug allergies, bacterial infection, and poor efficacy of antimicrobial therapy for non tuberculosis mycobacterium infection were observed. Thus, levofloxacin (LVFX), minocycline (MINO; was administered for 14 days), and streptomycin (SM) were used for treatment instead of CAM, EB, and RFP. The new therapeutic regimen alleviated the symptoms. Post-admission, Mycobacterium haemophilum was detected on the 38th day and the patient was discharged on the 57th day. Based on the antimicrobial susceptibility test results on the 65th day post-admission, CAM was prescribed with LVFX and SM. Treatment with these three drugs has been continued for over a year and no recurrence of symptoms has been observed. As there are few reports on M. haemophilum infections in Japan, it is necessary to accumulate the relevant case reports.
