Abstract A questionnaire survey was conducted in order to assess the current state of mycological examination in clinical laboratories of Japan in August 2010, and the chronological changes were assessed by comparison with similar survey data obtained in 1994 and 2002. Answers were returned by 172, 167 and 220 clinical laboratories in 2010, 2002 and 1994, respectively. Laboratories in which the incubation period for clinical specimens was considered deficient (2 or 3 days' incubation for internal medicine specimens, or 7 days' incubation for dermatological specimens), accounted for 19-50% of respondents in the three surveys, with the rate being highest in 2010 (47% for dermatological specimens and 50% for internal medicine specimens). CHROMagarTM Candida, a differential and selective medium for Candida spp., was used for isolation of clinical specimens in 75% and 81% of clinical laboratories in 2002 and 2010, respectively. Detection of (1→3)-β-D-glucan in blood for serological diagnosis of visceral mycoses was performed at 87% and 94% of laboratories in 2002 and 2010, respectively. Susceptibility testing for antifungal agents was ordered by clinicians in 31%, 48% and 59% of laboratories in 1994, 2002 and 2010, respectively. Between 1994 and 2010, serological diagnostic testing, use of differential and selective media, and susceptibility testing for antifungal agents tended to be performed more frequently in clinical laboratories, while the incubation period has decreased.
