Abstract We encountered a case of continuous ambulatory peritoneal dialysis (CAPD) peritonitis by Tsukamurella pulmonis. In June 2008, a 67-year-old male receiving CAPD lost the cap of his CAPD tube on the bathroom floor and recapped it after washing with shower water. Subsequently, turbidity appeared in his CAPD drainage, and abdominal pain was exhibited. In gram staining of the CAPD drainage subjected to a bacteriological examination, phagocytotic images of gram-positive bacillus were observed as well as a large number of leukocytes. By the gram staining observations, it was diagnosed as peritonitis associated with CAPD. Isolated strain was suspected as one of Tsukamurella spp. from cultured colonies observations and identified as T. pulmonis by phylogenetic analysis of 16S rRNA base sequence. He was successfully treated with the therapy involved oral administration of sulfamethoxazole-trimethoprim and intraperitoneal injection of amikacin, followed by oral administration of levofloxacin and RFP. This is the first case report of peritonitis associated with CAPD in Japan, while several cases like this were reported in other countries. To identify bacterial species in CAPD drainage, the careful observation under the consideration of the environmental bacteria including T. pulmonis are necessary.
