Abstract Streptococcus agalactiae (group B streptococci: GBS) causes serious infections in neonates, and elderly people. Penicillins are the first-line drug for the treatment of GBS infections, since all clinically isolated GBS have been considered uniformly susceptible to β-lactams, including penicillins. Recently, GBS isolates with reduced penicillin susceptibility (PRGBS) were reported, then we investigated the PRGBS in clinical specimens of our hospital. There was no PRGBS among 72 GBS isolates, however, 33 GBS isolates formed colonies in the growth-inhibition zone around ceftibuten disk. We sequenced PBP2X genes of 6 GBS isolates randomly chosen from these 33 isolates. Amino acid substitutions, S353Y, I373N, T394I and F399V were found in their PBP2X. These substitutions are identical or found in neighboring positions to those reported in PBP2X of PRGBS, though neither V405A nor Q557E, key substitutions of PRGBS, was found. In this study, we found the amino acid substitutions of PBP in GBS are diverse, and this observation may indicate the presence of prodromal stage for PRGBS.
