Abstract Leptotrichia trevisanii is an opportunistic pathogen, which is difficult to be identified by routine laboratory test, and 16SrRNA sequencing is necessary for the species identification. We report a case of bacteremia caused by L. trevisanii. The patient was transferred to our hospital because he presented symptoms of aspiration pneumonia during the course of outpatient chemotherapy for esophageal cancer. Bacterial growth was observed in 2 sets of blood cultures after 4 days incubation. Isolated strains from those blood cultures were all identified as L. trevisanii by MALDI-TOFMS (Bruker MALDI Biotyper). Later, the result was confirmed by 16SrRNA sequencing. MALDI-TOFMS is suggested to be a quick and useful identification tool for this species.
