Abstract We evaluated the modified semi-alkaline-protease (SAP)-2%NaOH decontamination procedures by using a decreased amount of expectorated sputum in the aerosol-free 2 ml micro-centrifuge tube with high speed refrigerated micro centrifuge. A total of 206 sputum specimens obtained in NHO Kinki-Chuo Chest Medical Center from July to August 2014 were subjected to two decontamination methods (SAP-NALC/NaOH vs modified SAP-NaOH). All specimens were divided into two portions after concentrating the sediments processed by SAP, then decontaminated, and inoculated into MGIT. Any tubes were incubated by BACTEC MGIT-960 and monitored for up to forty-second days. Comparing these decontamination procedures, the time of the recovery of rapid growing mycobacteria strains in the modified SAP-NaOH was significantly more increased. Of these, 39 specimens processed by the SAP-NALC/NaOH were positive for growth of mycobacteria, and similarly 46 specimens processed by the modified SAP-NaOH (p>0.05) were positive. Nineteen specimens in the modified SAP-NaOH and the 18 specimens in the SAP-NALC/NaOH were positive by TaqManPCR. The decontamination rate was 1.5% in SAP-NALC/NaOH and 2.4% in modified SAP-NaOH, however the difference was statistically not significant (p>0.05). We verified that the modified SAP-NaOH was an alternative method suitable for the digestion and decontamination procedure, and was useful for the isolation and detection of mycobacteria as well, including DNA extraction and purification, and offered the increased efficiency and validity in complicated centrifuge operation. This modified decontamination also features an easy-to-use design with time-saving mode by a compact body centrifuge machine.
