Abstract Streptococcus pneumoniae frequently causes community-acquired pneumonia, meningitis, bacteremia, and otitis media. Due to the effect of the pneumococcal conjugate vaccine, there has been a clear decrease in cases of invasive pneumococcal disease (IPD) caused by serotypes contained in the vaccine. On the other hand, cases of IPD due to serotypes not included in the vaccine are increasing. This time we have experienced a case of purulent meningitis caused by S. pneumoniae serotype 23B. It has already been reported that cases of serotype 23B were detected in Europe and North America, etc., but we believe that no other cases have been reported in Japan before. Such new non-vaccine type (NVT) strains may increase in Japan in the future, and we should pay attention to these trends.
