Abstract We report here a case of Legionella pneumonia that was diagnosed by Gram staining of a suction sputum sample. The patient, a 79-year-old woman with no underlying disease, was found drowning in a bathtub at home, and was brought to St. Marianna university school of medicine yokohama city seibu hospital, as she was in a state of impaired consciousness. Considering the impaired state of consciousness, weakness due to drowning, and dehydration due to an influenza virus infection, hypoxic encephalopathy was diagnosed. Pneumonia reported at the time of hospitalization, was ameliorated after the administration of antibiotics. However, on day 9 of admission, the patient's condition was newly consolidated and respiratory status deterioration was noted; Legionella pneumophila was detected on Gram staining the suction sputum sampled at that time. After sharing this information with the laboratory and the clinicians, administration of levofloxacin was initiated. The growth of Legionella pneumophila sero groups 5 and 6 was detected on B-CYEα and WYO-α agar media after 72 h of incubation. In this case, we identified the presence of an aggressive, difficult-to-detect Legionella sp. by Gram staining. Thus, we were able to contribute to the diagnosis and treatment of Legionella pneumonia by sharing relevant information with the laboratory and the clinicians.
