Abstract |
In recent years, the spread of OXA-48-like carbapenemase-producing Enterobacteriaceae has been a public health concern mainly among European countries. In Japan, OXA-48-like carbapenemase-producing Enterobacteriaceae have only been reported so far in patients with a history of international travel. These were considered imported cases. However, here we report the first domestic case, as the patient and his family had no history of international travel. The patient underwent brain surgery for acute cerebral infarction in April 2012, but due to poor postoperative prognosis, he repeated hospitalization for about two years. Carbapenem-susceptible extended-spectrum β-lactamase (ESBL)-producing Klebsiella pneumoniae has been consistently isolated since his first hospitalization in 2012, but in December 2013, carbapenem-resistant K. pneumoniae was isolated from a urine sample obtained from his urethral catheter. The isolate was confirmed to harbor blaOXA-48-like by PCR. In addition, further analysis revealed that carbapenem-susceptible ESBL-producing K. pneumoniae isolated from this patient in 2012 also harbored blaOXA-48-like. It is possible that local transmission of isolates with blaOXA-48-like has already occurred in Japan. |
