Abstract Several previous studies supported the potential relationship between the development of late-onset psychosis and several tauopathies, i.e., argyrophilic grain disease (AGD), senile dementia of the neurofibrillary tangle type (SD-NFT), and progressive supranuclear palsy (PSP). It was reported that AGD may be associated with the development of late-onset psychosis, as well as the development of late-onset bipolar disorder, the severity of post-stroke depression, and the socioeconomic status. Tau accumulation in the nucleus accumbens in SD-NFT cases was significantly more severe than that in Braak stage-matched Alzheimer's disease cases, and SD-NFT cases are known to often show delusion in life. Severe tau accumulation in the nucleus accumbens was also associated with delusion in cases of Alzheimer's disease. It was reported that 4.8% of patients with PSP showed delusion within three years after the onset. The development of surrogate markers for molecular pathologies, as well as the accumulation of clinicopathological findings, may be necessary for better understanding of the pathogenic background of psychosis that occurs in middle-to-late life.
