Abstract Frontotemporal dementia (FTD) with right temporal lobe-predominant atrophy has been termed "right predominant semantic dementia,""right temporal variant of FTD,""semantic behavioral variant FTD," among other names. Although there are no consistent criteria or nomenclature for FTD with right temporal lobe-predominant atrophy, its behavioral symptoms are reportedly more severe than those in FTD with left temporal lobe-predominant atrophy. Our report and several others from Europe and the United States have revealed that disinhibition, apathy, loss of empathy, and complex stereotypic behaviors are prominent in the early stages of the disease. In Western countries, the frequency of delusions and hallucinations is relatively high, and hyper-religiosity has been often reported. Imaging studies have suggested that the right orbitofrontal cortex, right insula, and right anteromedial temporal areas are involved in disinhibition and stereotypic behaviors in FTD, and the same regions may be involved in these symptoms in FTD with right temporal lobe-predominant atrophy. However, few studies on the brain basis of the behavioral symptoms observed in patients with FTD with right temporal lobe-predominant atrophy have been published, and further research is necessary.
