Abstract |
We report results of extraoperative cortical stimulation in the language-dominant hemisphere to clarify the neural organization of speech. Arrays of subdural electrodes were placed in patients with intractable epilepsy. Electrical stimulation was applied during various language tasks including naming, repetition, reading aloud, listening comprehension and reading comprehension. Cortical stimulation of both the inferior frontal and superior temporal cortex markedly interfered with language output in some patients. At some cortical language sites in this region, cortical stimulation also evoked language comprehension deficits. Cortical stimulation at two adjacent sites just 1 cm apart produced different pattern of language deficits. Cortical stimulation of a very short period at the different timing to the presentation of characters revealed that electrical stimulation interfered with different stages of reading at each site. Our results indicated that both language output and input are based on distributed parallel networks over peri-Sylvian area in the language-dominant hemisphere. |
