Abstract We report a therapeutic approach to a case of anosognosia and its subsequent clinical process. In this case, the patient, NK, denied that his left arm was a part of him and acknowledged the presence of a supernumerary phantom limb. He also denied experiencing hemiparesis following right putaminal hemorrhage. When he was made to look at himself in a mirror from the back, he seemed to be able to recognize a discrepancy between what he physically felt and what he saw in the mirror. However, he subsequently exhibited the general symptoms of anosognosia again. Assuming that the right middle cerebral artery territory- including the right premotor cortex area, the intraparietal cortex area, and the insular cortex- is a key area that is dominantly responsible for the "network which is important for recognizing self-awareness and functioning of one's own limbs", it can be considered that an extensive disconnection in this network as a result of right subcortical damage would render the therapeutic approach difficult.
