Abstract Studies using structural brain imaging techniques such as magnetic resonance imaging (MRI) have provided a number of important clues to help our understanding of the pathophysiology of schizophrenia. However clinical implications of the MRI findings in schizophrenia are not sufficiently clear. This manuscript summarizes the clinical implications of progressive brain changes in schizophrenia with respect to (1) clinical symptom severity, (2) treatment responsiveness and outcome, (3) relapse, (4) manifestation of overt psychosis, and (5) impact of antipsychotic treatment. Our attempts to apply structural MRI to adjunctive clinical diagnosis of schizophrenia are also described.
