Abstract Some different mechanisms of unilateral neglect (UN) are discussed. Because of their fixation difficulty, UN patients shows pseude hemianopia on visual field examination. Visual extinction is a sensitive task for detecting UN. Attention disorder of contralesional side is thought to be major cause for UN, and the disorder is thought to be ocurred by the one sided disruption of reticular formation system. Representation level neglect is also important for explaining UN in spontaneous drawing. And UN in motor expression is called as hemi-akinesia. Then, behavior disorders in right hemisphare damaged patients (RHDs) are discussed. Emotional fluttering and indifference are conspicuous among RHDs, and attention and arousal disorders are in the background for these disorders. Reduplication and Capgras syndrome are also characteristic in RHDs. Confabulation is common in these syndromes, and they have relationships with memory, attention, including UN and conceptual disorders. Anosognosia due to RHD are thought to be UN for their own body. Paraliguistic disorder are also occured in RHDs. They cannot infere hidden intentions in the discourse. Aprosodia is also important in communication disorders in RHDs.
