Abstract In this review article, cognitive and behavioral disturbances associated with subcortical lesions will be discussed from three perspectives. First, key neuroanatomical network concepts, which are the frontal-subcortical circuits, ascending neuromodulatory systems and thalamo-medial temporal memory networks, will be presented. Second, the concept of "subcortical dementia" will be revisited to provide an integrative framework for understanding cognitive and behavioral disturbances arising from variety of subcortical lesions. Finally, I will focus on individual symptoms associated with subcortical lesions, i.e., apathy, disinhibition, cognitive slowness, hallucinations and memory impairment.
