Abstract The mental state examination is a part of neurological examinations, which is a systematic method for examining a patient's emotion, behavior and cognitive functions. The interview on problems in daily activities provides cues about the underlying cognitive dysfunction. Higher cortical functions are hierarchically organized; the basic functions (attention, mood, etc.), individual cognitive functions (language, memory, etc.), and the integrative functions (executive functions, social functions, etc.). We should consider which level is impaired and how dysfunctions at one level would affect the functions at the higher level. We could briefly assess each domain in a mental status examination using screening tests and some tasks of apraxia and visuospatial functions. When examining the mental state, we should always consider the premorbid level of patient's abilities.
