Abstract A 55-year-old man who had a history of myelopathy a year before recurrence experienced acute behavioral disturbance. Neurological examination on admission one and a half months after the onset revealed a left homonymous hemianopia, memory disturbance, and transcortical sensory aphasia. At the beginning of the interview by attending doctors, he made a repetitive speech with almost same words and contents including his symptoms and personal problems. These verbal stereotypies were observed on every visit. He had persisted in daily schedule and often became angry at change of scheduled events. On MRI, recent lesions were located in the deep white matter in the right inferior frontal, superior to inferior temporal, and angular gyri and border-zone between the right temporo-occipital lobes, and the left supramarginal and superior temporal gyri. He was diagnosed as recurrence of multiple sclerosis. His neurological deficits were gradually improved. His repetitive speech was compatible to "Spieluhrsymptom".
