Abstract In order to clarify a certain cognitive processing, it is important the "pure" case which means the case with selective impairment of that function. In the literatures, nine cases of pure amusia were reported. The common lesion existed at cortical and subcortical region of right superior and middle temporal gyrus. The musical anhedonia means the acquired and selective loss of musical emotion. Including two cases by the author, five cases of musical anhedonia were reported. Most of them were damaged at cortical and subcortical region of right posterior portion of insula and superior temporal gyrus. There are some cases who preserved the musical emotion even under the condition of the impairment of perception and recognition of music, so it is reasonably concluded that these two mental processing exist independently in the brain partially at least. These findings were congruent with those of the activation study using PET and fMRI. We need to do and interpret the results of the experiment based on the understandings about the characteristics and limitation of the case and activation studies.
