Abstract The Act for Promotion of Employment of Persons with Disabilities requires all employers to employ persons with disabilities at the statutory rate or more. The statutory rate for private companies is 2.2% as of 2020, and is scheduled to raised up to 2.3% in April, 2021. The demands for persons with disabilities who can work and continue to work will further increase in Japanese labor market. This paper summarizes the systems and supporting facilities for employment of persons with disabilities and describes a case of a left putaminal hemorrhage with cognitive dysfunction. A hospital for recovery rehabilitation, an employment support center for persons with disabilities, and a transition support facility for employment provided support services in each phase, and the case finally got a new employment 4 years after onset. This paper reports the period of 4-year from the perspective of medical and welfare support for employment and multi-facility cooperation.
