Abstract The patient with pure word dumbness is unable to speak, though he can read, write and comprehend spoken language. Pure word dumbness is often accompanied by slight agraphia. When pure word dumbness is accompanied by moderate or severe agraphia, it is classified as Broca's aphasia (Lichtheim 1885). It is necessary to clarify whether the lesion of the lower part of the precentral gyrus causes pure word dumbness or Broca's aphasia.
The patient with pure word blindness is unable to read comprehend, though he can speak, write and comprehend spoken language. Pure word blindness is often accompanied by agraphia of difficult Chinese characters, which we rather think to be agraphia of low frequency words. The alexia with agraphia of low frequency words is caused by the lesion of the middle to the posterior part of the left medial occipito-temporal gyrus, the posterior part of the left lateral occipito-temporal gyrus, and the white matter which is above the two gyri.
