Abstract Aphasia is the most common cortical symptom in stroke patients. The incidence of aphasia was 14% of 4,313 acute brain infarction patients who was registered by Japan Standard Stroke Registration Study (JSSRS). Aphasia is important research theme but dementia becomes big social problem now. Frontal dysfunction is characteristic in vascular dementia in contrast to Alzheimer's disease. Recently, a new category "vascular depression" was presented, and it should be one of the organic depression. In higher brain function disorders of stroke patients, it should be considered emotion and executive function that is important in social life.
