Abstract We report the clinicopathological features of two autopsy cases of Pick's disease with Pick bodies presenting with apraxia of speech as an initial sign. We examined the responsible lesion for apraxia of speech using hemisphere and bisphere specimens, respectively. Cerebral cortical lesions was divided into three categories (slight, moderate, and severe). We elucidated severe lesions in the posterior portion of the left inferior frontal gyrus (pars opercularis) in the two autopsy cases. In contrast, the precentral lesions of the two cases were relatively moderate. Literature review concerning ten autopsy cases, including our two cases, showing the aphasia as the initial sign, disclosed that in eight cases of ten cases "motor aphasia" was the initial sign.
