Abstract Objective: To establish a questionnaire for executive dysfunction in activity of daily living (ADL) in Alzheimer's disease (AD). Methods and Results: We assessed 66 AD patients using the Alzheimer's Disease Assessment Scale (ADAS) and the Frontal Assessment Battery (FAB), and interviewed the informant of each patient using the Clinical Dementia Rating (CDR) and the Dysexecutive Questionnaire (DEX) from the Behavioral Assessment of the Dysexecutive Syndrome (BADS). The scores of the 13 of the 20 items in DEX were significantly correlated to each score of the two executive function tests, the ideational praxis of ADAS and FAB. The 13 items were employed for the DEX for dementia (DEX-D). The total score of the DEX-D was significantly correlated to the scores of word recall, orientation, ideational praxis of ADAS and the total score of FAB. A multiple correlation model which include the rating of CDR, the DEX-D score and each cognitive score showed the significant correlation between the DEX-D and each cognitive scores after eliminationg the effect of CDR. Conclusion: DEX-D maybe useful questionnaire for executive dysfunction in ADL in AD.
