Abstract |
Objectives: To estimate and compare the annual rate of cognitive decline in patients with Dementia with Lewy bodies (DLB) and Alzheimer's disease (AD) in a Japanese memory clinic. Methods: We studied 86 AD patients and 29 DLB patients in the memory clinic of Niigata Rehabilitation Hospital. All the patients scored 10 or more on the initial assessment with Mini-mental state examination (MMSE). The mean age (±SD) was 78.7±6.1 in AD and 81.2±7.1 in DLB. We assessed the patients with MMSE and Alzheimer's Disease Assessment Scale (ADAS), and repeated the assessment one year after. The annual rates of change on the scores of MMSE and ADAS were compared using the analyses of variance and covariance. Result: The mean±SD (range) of the annual rate of MMSE change was -0.6±2.7 (-7.0 to 6.6) in AD, and -0.4±3.3 (-9.9 to 7.4) in DLB. There were no significant differences in the rate of decline between two groups. The mean±SD (range) of ADAS change was 1.1±4.9 (-8.2 to 14.8) in AD, and 4.0±7.6 (-14.6 to 22.7) in DLB. DLB patients showed a significantly faster decline on the annual ADAS change (F=5.6, P<.05). The result was still significant after excluding the effects of overall cognitive severity and medication of donepezil using a two-way analysis of covariance. Conclusion: This study suggests that DLB patients show a faster cognitive decline when compared with AD. |
