要旨 Behavioural-variant frontotemporal dementia (bvFTD) is a progressive neurodegenerative syndrome which is characterised by atrophy of the mesial, orbitofrontal lobe and insula cortices. Clinically, it is characterised by changes in behaviour and personality. The challenges with diagnosis of bvFTD, the variable profile on neuropsychological assessment and the high degree of misdiagnosis, has led to a large body of research focused on developing better diagnostic tests which are sensitive to bvFTD. In this respect, assessment of social cognition has become increasingly recognised and is showing promise in light of the overlap between brain atrophy in bvFTD, regions recognised as forming part of the "social brain", and the symptomatology, which can be seen as reflecting progressive loss of social cognition capacity. In this review, we discuss current findings regarding profiles of social cognition in bvFTD, with a focus on emotion recognition and empathy. The studies discussed demonstrate that it is now clear that bvFTD patients demonstrate generalised deficits in emotion recognition irrespective of modality. Furthermore, capacity for empathy is profoundly reduced, as demonstrated by both informant-based questionnaires and objective experimental assessment. Finally, we discuss outstanding issues in the field, and how future research can add to our evolving knowledge of social cognition in bvFTD, with a focus on clinical assessment and potential issues surrounding cross-cultural assessment of social cognition. We conclude by recognising that the incorporation of social cognition assessment informs diagnosis, prognosis and management of patients with bvFTD and should form a central part of the neuropsychological assessment of patients where this diagnosis is suspected.
